Yes, it is possible to get a quality score of 10 but this usually happens after you've been actively running campaigns for some time. We have had a few campaigns with 10s and several with 9s.
The main factor in campaign quality score is high click-through rates and for Facebook (more than other ad networks) the ad image contributes to about 90% of this. The more images you test, the higher the likelihood of getting a 'hit' i.e. one with significantly higher CTRs than all the others.
The other factor is lack of complaints. If your image has "shock and awe" value and gets a high CTR but receives complaints your quality score will quickly fall.
